The following resources apply to all HIE participants, regardless of the HIE solutions your organization is utilizing.

HIE Security Best Practices and Appropriate Use

This video explains HIE security best practices and guidelines for appropriate use of data available in the HIE. All HIE participant users are encouraged to watch this training video.

All users from an organization participating in the Contexture HIE must review and agree to this HIE Appropriate Use Agreement. Signed agreements should be filed in each user’s employment record.

PatientCare 360 Navigation & Features

This training video is designed to help you maximize your experience with the new portal, providing an overview of key features and workflows. While watching this training video is highly recommended for all PatientCare 360 Portal users, it is not required to gain access.

This training is required for patient Part 2 data access in the Contexture HIE portal, PatientCare 360. This video provides an overview of Part 2 data, what it is, how it’s protected, and how you can access Part 2 data in PatientCare 360.

HIE participants must have a process in place to notify patients when they visit next, that their information is shared through the HIE. We put together the following resources to help your staff set up efficient processes and keep your patients informed.

contexture check Notice of Health Information Practices

HIE participants are required to provide notice of participation to all patients. This notice is an important step towards ensuring patients understand the HIE and their rights under state and federal law. It helps patients understand that participation is free and improves their care coordination by securely sharing health information. This Notice is required in Arizona. Other participants are encouraged to use this Notice but may instead incorporate the Notice language into their own Notice of Privacy Practices.

Patient Rights Forms

The following forms allow patients to exercise their rights regarding participation in the HIE. Please download these forms and make them available to your patients upon request.

HIE Opt-Out and Opt Back-In Forms 

Your patients have the right to-opt out of the HIE under state and federal laws. To opt-out or opt back-in to the HIE, a patient must complete the applicable form below and submit it to their healthcare provider’s office.

Part 2 TPO Consent Form

This form authorizes the sharing of a patient’s Part 2 data with health care providers and health plans and for the purposes permitted by law. Patients must complete this form to consent to data sharing. Once signed, HIE participants should upload the form to the PatientCare 360 Portal. After a participant has completed their training and are provisioned Part 2 access, they can then upload consent forms and the Part 2 data, if any, will be available immediately thereafter if they are of the correct user role.

To access Part 2 data through the HIE, you must use Contexture’s new Part 2 consent form.

  • The new platform only accepts the updated form; older (legacy) forms will not work.
  • Patients must complete and submit the new consent form, and providers must upload it into the HIE.
  • The consent form expires every 2 years, so patients will need to renew it when it expires.

Health Information Request Form

(For Arizona use only)

This form is intended for use in Arizona only. Patients have the right to request a copy of their health information available through the HIE, as well as a record of who has accessed it within the past three years. A patient must complete the form, and submit it to their healthcare provider’s office. The provider must fulfill the request directly.

(Only Applicable to AZ Participants)

State law governing health information exchange in Arizona. Covers patient rights (opt-out, amendments, breach notifications) and compliance requirements for HIE participants.

Federal Law: 42 CFR Part 2

Federal regulation protecting the confidentiality of substance use disorder patient records. Requires patient consent for disclosure and sets strict rules for handling and redisclosure of Part 2 data.
