Contexture Health Information Exchange:
Patient Opt-In / Opt-Out Information
Contexture’s health information exchange (HIE) serves both Arizona and Colorado and offers many benefits to patients. Your participation is voluntary — you get to choose whether you want your information included.
Your Right to Choose
If your healthcare providers participate in Contexture’s HIE, they are required to notify you of their participation. This notice may accompany the provider’s HIPAA privacy notification during your visit.
You can decide whether your health information is shared through Contexture’s HIE. If you choose not to share your information, this is called “opting out.” By opting out, your health information will not be accessible through Contexture’s HIE unless the sharing is required by law.
We encourage you to review our FAQ for Patients here and talk to your doctor before making a decision to opt out. Opting out may limit access to your health records in emergency or urgent care situations, which could impact your treatment.
Your health information shared with the HIE is securely guarded and only accessible to your care teams who work for authorized entities who contract with Contexture. Review our FAQ here to learn more about who can access your information and how the HIE operates.
After opting out, your health information may still be shared through other legal means outside of Contexture’s HIE including fax, mail, and other health information networks your provider may participate in.
How to Opt Out
When you visit a doctor’s office, hospital, or clinic that is participating in the HIE and do not want your health information included, simply tell the doctor or other healthcare provider that you wish to opt out of the Contexture HIE.
Once this form is processed, your health records and information will not be accessible through the HIE to your care teams.
How to Opt Back In
If you have previously opted out, you may opt back in at any time. To opt back in to the HIE, tell your doctor or other healthcare provider when you visit a doctor’s office, hospital or clinic that is participating in the HIE.
