A New Leaf is the recipient of the 2023 Contexture Excellence Award – Organization. This award is presented to an organization that demonstrates innovative uses of health information technology, and A New Leaf is a leader in its utilization of CommunityCares, the single, statewide referral system to address social determinants of health (SDOH) needs in Arizona.
A New Leaf has been serving the Arizona Valley since 1971 with housing, shelter, health and other SDOH services to families and individuals, going above and beyond sending patient information to community-based organizations (CBOs) to help address patients’ needs outside of clinical workflows. The nonprofit also provides a plethora of valuable community programs that support patients’ social needs. From trauma recovery and therapeutic after-school programs to offering up to 120 days of safe, temporary housing for victims and their children, A New Leaf provides life-changing programs that help households transition from crisis to stability, and these offerings and capabilities are all made possible through the organization’s willingness to embrace and leverage new technologies for the betterment of Arizonans.
