PatientCare 360 Portal provides convenient, secure access to patient data shared within the HIE. With Portal, clinicians can access and query patients’ longitudinal physical and behavioral health history to support clinical decisions at the point of care, and office staff can easily obtain details necessary for billing, scheduling and other non-clinical workflow.

Notifications allow Contexture participants to track and monitor important patient health information across the care continuum, such as hospital admissions, discharges and transfers, lab reports and clinical test results. Real-time or daily notifications enable proactive patient follow-ups, care coordination and timely care decisions that improve outcomes and control costs.

The Data Sender family of solutions allows participants to seamlessly and securely send data from their EHR to the Contexture HIE. Data made accessible to other HIE participants reduces administrative burden and supports optimal patient care. Participants can leverage Data Sender solutions to allow Contexture to complete reporting on their behalf, such as for meeting public health requirements, earning value-based payments, and programs.

The Direct Secure Message solution allows healthcare providers and their support staff to send and receive HIPAA-compliant messages to support care coordination and improve patient outcomes. The solution enables rapid delivery of information such as treatment plans and updates, test results, referrals, transition of care and care summary documents.

Results solutions deliver patient results – including laboratory, pathology, radiology and transcribed reports— in the manner that best meets the provider’s workflow. This allows rapid patient follow-up to improve quality of care and outcomes, while also saving time. Results Delivery seamlessly delivers patient results directly into the EHR or through other secure channels. Quick Results delivers patient results to an inbox within Portal.

Reports provide Contexture participants with the up-to-date information necessary for care coordination, population health management, quality reporting and data analytics. Leveraging data from the thousands of health systems, laboratories, imaging centers and ambulatory care providers in Arizona and Colorado, Contexture offers reports in a variety of standard formats, as well as custom reports based on an individual participant’s needs.

Smart Summaries combine, normalize and deduplicate data from across the Contexture HIE to create a one-stop-shop for reviewing a patient’s medical and social history, including clinical data, diagnoses, procedures, lab results and demographics. Smart Summaries save time, support better decision-making and increase efficiency.

History provides robust historical patient data to support decision-making and reporting. Clinical lookbacks can be used to analyze patient and population histories to assign risk scores, inform reporting for value-based contracts or quality reporting programs, and improve care management and disease prevention programs.
