Helping Colorado Hospitals Succeed in the Hospital Transformation Program

The Colorado Hospital Transformation Program aims to improve care quality, demonstrate meaningful community engagement, and improve health outcomes over time. Contexture can help hospitals participate in this program and support these meaningful goals for Colorado.

How Contexture Supports Core Hospital Transformation Measures 

We help hospitals meet HTP measures through timely information sharing, coordinated care planning, and automated data sharing with Regional Accountable Entities (RAEs). For hospitals sending data to the HIE, we transmit your data directly to RAEs via Notifications configured for HTP core measures, eliminating manual work for your staff. Each quarter, we provide a summary report to participating hospitals detailing what data was sent to the RAEs on their behalf.  

Our Quality Improvement Team is ready to help your staff audit and optimize your program performance with technical, workflow, and operations assistance whenever you need.

HTP Measures Supported by Contexture 

Statewide Core Populations 1 (SW-CP1)
Notifications sent to RAEs for positive results on Social Needs screenings. 

Core Population 1 (CP1) 
Notifications to care teams and RAEs for readmissions among patients with chronic conditions. 

Core Population 6 (CP6)
Notifications to RAEs for positive screenings for perinatal or postpartum depression and anxiety. 

Statewide Behavioral Health 1 (SW-BH1) 
Discharge notifications to RAEs for patients diagnosed with mental illness or substance use disorder (SUD) from hospitals or emergency departments. 

Reducing Avoidable Hospital 1 (RAH1) 
Notification to RAEs when a follow-up appointment with a clinician is scheduled prior to discharge.
