What is an eConsult?

An eConsult is an asynchronous physician-to-physician conversation between a primary care provider (PCP) and specialist to aid patient treatment. The PCP signs into a web-based platform, pulls up the patient’s information and asks the question. The specialists review the case and respond with additional questions or their recommendation.

Example Workflow

  1. The patient presents with a medical issue that may benefit from a specialist’s guidance.
  2. PCP educates the patient on the benefits and possible risks associated with using an eConsult and gains their consent.
  3. Submit relevant:
    • Medical history
    • Labs or other test results
    • The clinical question
  4. The specialist reviews the submitted information and provides a detailed response, which may include:
    • Additional questions
    • Clinical recommendation
  5. The PCP reviews the specialist’s response, asks any follow-up questions, or closes the eConsult.

To help your practice integrate eConsults smoothly, it’s important to understand the roles available in the platform and the responsibilities associated with each. The descriptions below clarify who can perform which tasks, so you can design workflows that best support your team and optimize patient care.

Provider Role (MD, DO, NP, PA)

  • Primary Function: Submit, reply to, and close eConsults.
  • Workflow: Providers manage the full lifecycle of an eConsult, from initiation through final resolution.

Staff Draft Role (MA, LPN, other clinical support staff)

  • Primary Function: Draft eConsults on behalf of a provider.
  • Workflow: Staff create a draft eConsult and assign it to a specific provider.
  • Once drafted:
    • The provider receives an email notification.
    • The provider reviews, updates as needed and submits the eConsult.

Org Admin Role (Practice Manager, Billing Team, Site Leadership)

Primary Function: Oversee and manage eConsult activity across designated practice sites.
Capabilities:

  • Review eConsults generated within their assigned locations.
  • Reassign eConsults to covering providers when needed.
  • Run reports to track:
    • Total, in process, closed, or scheduled consults.
    • PCP activity, including billing related reporting.

RMHP partnered with ConferMED, a national leading eConsult company developed by Primary Care Providers. The specialists are trained and experienced in working with Primary Care Providers. All specialists are board certified and credentialed in Colorado.

If a face-to-face visit is recommended or preferred, the PCP follows their standard referral procedures. Specialist responses will include pre-visit guidance such as recommended work up, guidance on length of time that the patient can safely wait for a follow-up visit, and recommendations for short-term care measures (medication, therapy, etc.).

Practices can bill RMHP and State Medicaid for CPT code 99452 (excluding Federally Qualified Health Centers (FQHC) and Rural Health Centers (RHC)).

99452 is an interprofessional telephone/internet/electronic health assessment, and management service(s) are provided by a treating/requesting physician or other qualified health care professional for a duration of 30 minutes.

The following requirements must be met to bill for the code:

  • Billing Practitioner: MD, DO, NP, PA.
  • Documentation: Documentation of consultation must be in the patient’s medical record.
  • Consent: Verbal patient consent must be documented in the patient’s medical record for each consultation, including the reason for the request.
  • Time: >16 min spent on interprofessional consultation including preparation of consult, review of guidance and communication with the patient. This must be documented in the patient’s medical record
  • Cost Sharing: There is no cost sharing for RMHP members eligible for this service

Consent is required to be obtained and documented in the patient record to bill for the eConsult. Consent should include the potential benefits and potential risks of using an eConsult. Example benefits may include improved access and timely expertise, while potential risks may include insufficient information, incomplete records or technical delays.
